Experience/Service - Data Fields

The following is an all-inclusive list of data fields for all experience/service types. No description or examples are provided for self-explanatory data fields; not all data fields apply to every Experience/Service type.

Academic Appointment Type

Applies only to Academic Appointments and is explained in detail under Academic Appointment Type.

 

Organization/Institution

Generally pre-populated as the way your institution designates its official title. Value can be overridden if a different organization/institution applies.

 

Branch of Military Service

Applies only to Military or Other Governmental Service type.

 

Committee Name

Applies only to Institutional Committee Activities. Enter name(s) of committees on which you have served where institution is the current institution.

 

Academic Rank, Title, Endowment Title, Position Held, Rank/Position

Required field with the exception of Consultantships.

For academic rank:

If academic rank is Assistant Professor and academic appointment type is Ad Interim, then select Other and enter Assistant Professor, Ad Interim as text.

 

If academic rank is Assistant Professor and academic appointment type is Adjunct, then select Other and enter Adjunct Assistant Professor.

Endowment Type

Applies only to Endowed Positions.

 

Description

Applies only to Consultantships. Enter a brief description of your duties, limited to 255 characters.

 

Division

Drop list consisting of the most common divisions at your Institution. If there is no exact match, select Other and enter the division preceded by Division of.

Drop list consisting of the most common departments at research hospitals. This is a lookup table that can be edited by a site administrator.  If there is no exact match, select Other and enter the department preceded by Department of. Note: the department selected must be compatible with the division.

 

Start Date

Format: MM/YYYY or YYYY

Year is required; month is optional.

If only one date is available on the current CV, enter it under Start Date.

 

End Date

Format: MM/YYYY or YYYY

Month and Year are optional.

 

Location

Applies only to Consultantships. Enter city, state, and country.

 

City

Address fields that follow apply to all, but Consultantships.

 

State

 

 

Province

 

 

Country

 

 

Sequence for Titles to Appear under Present Title and Affiliation

Note: this description applies to all ranks, titles, and positions, but for simplicity, we will refer only to title.

 

Only selected titles will be displayed under Present Title & Affiliation (all titles will be printed on the CV Report under Experience/Service).

 

The drop list for this field consists of Do not show under Present Title and Affiliation followed by the numbers 1-20.

 

To suppress printing of a title under Present Title & Affiliation, leave the default, Do Not Show under Present Title and Affiliation.

 

To print a title under Present Title & Affiliation, select the number indicating the sequence in which to display the title. (see Examples)

To display titles in the following order:

 

Professor, Radiation Oncology

 

Director and Service Chief, Breast Cancer Radiation Oncology Program

 

Clinical Medical Director, Mays Clinic

 

From the drop list, select “1” for the first title, “2” for the second title and “3” for the third title.
