Faculty Member Information – Data Fields

The following is a list of data fields included in this section. No description or examples are provided for self-explanatory data fields.

Faculty Member Information
Data Field	Description	Examples
User ID	Employee ID, imported by FIS.
First Name
Middle Initial(s)	Middle initial(s) as you wish them displayed on the top of faculty member’s CV. The standard is: period after each initial with no space in between.	B., J.R.
Last Name
Suffix	Follows faculty’s full name and provides additional identifying information.	Jr., Sr., 2nd, III
Degrees and Honors	Degrees and honors entered in the order in which you wish them displayed after faculty member’s name.	MD, PHD, MS, FACS, FACR
Email	Valid email address required. Used to email password when forgotten.
NIH eRA Commons	Unique identifier assigned by NIH to a faculty member allowing him/her access to information on the NIH eRA Commons web site.
Do not show on CV	Yes No This field applies to all home information. The default is NO. If you wish home information to be printed on your CV, answer Yes.

Province	Enter only when Country of Citizenship is not the United States.

Data Field	Description	Examples
Visa Status	Consists of a drop list of valid visa status entries and is displayed only if Country of Citizenship is Other Country.	Valid entries: F, H-1B, J1, O, Pending Permanent Residency, Permanent Resident
Country	Consists of a drop list of universally acceptable names of countries and is displayed only if Country of Citizenship is Other Country.
Last CV Update	Read-only field displaying date of last CV update. To update to today’s date, click Date of Last CV Update on left navigation bar.
